The Nigeria Air Force (NAF) has released the preliminary investigation report into the Mi-24B Helicopter that crashed on May 9 at the NAF Base in Port Harcourt, Rivers State, with the crews escaping unhurt.
The verdict of the report, which was disclosed in a statement signed Monday by the NAF Director of Public Relations and Information, Air Commodore Yusuf Anas, stated that the crash occurred due to a system failure.

Anas said the team of safety and engineering investigators who accessed the accident said it might have occurred as a result of a “tail rotor systems failure” in the helicopter.
The report further stated that “the helicopter engines and the gear box were in good serviceable condition at the time of the accident,” adding that mechanical machines are occasionally subject to failure as a result of system’s malfunction.
“The report also confirmed that the helicopter had flown 1,908 hours before the accident,” he said.
The NAF spokesman recalled that sequel to the helicopter accident, the Chief of Air Staff (CAS), Air Marshal Alex Badeh, immediately directed the grounding of the entire Mi-35P and Mi-24B helicopter fleet and ordered immediate investigation into their serviceability status.
He disclosed that following their certification by the investigators, the Mi-helicopter fleet had now been certified fit to resume flying.
